Facility is in need of a thorough cleaning, especially back storage room and restroom. Facility has 30 days to thoroughly clean facility or facility may be fined $500 for failure to comply with the Illinois Food Code.

A follow up will occur in 30 days to verify facility has been thoroughly cleaned.
